date of the child; (3) your relationship to the child (e.g., parent, guardian, teacher);

HOW DO YOU VIEW OR EDIT PERSONAL INFORMATION AND PREFERENCES?
Login and go to My Account to view and/or update any personal information provided,
such as mailing and billing addresses, e-mail addresses, or musical interests and
roles. If you do not wish to have personal information shared with advertisers or
non-essential business partners, you may select the option “Check this box if you
do not want MakeMusic to share your contact information…”. Note we must be able
to share your personal information with essential business partners, such as Visa
or MasterCard, in order to provide you with services. If you do not wish to receive
e-mail information from SmartMusic.com (including, but not limited to, information
on critical issues and technical updates) de-select the option “Yes, keep me informed
